Quarterly Treatment Capacity Survey

Please see important notice from ADHS regarding the quarterly treatment capacity survey.
QUARTERLY SUBSTANCE ABUSE TREATMENT REPORTING REQUIREMENTS
This is for the quarter ending March 31, 2019, and is due April 12.
Arizona Revised Statute §36-109 requires that for each quarter, each hospital, health care facility and outpatient substance abuse treatment providers that provides substance abuse treatment submit to the Arizona Department of Health Services the following information using the below Survey Monkey link: https://www.surveymonkey.com/r/ADHS-SUDT-Survey-Q1-2019
- Name and address of the hospital or health care facility,
- The type of hospital or health care facility
- The number of available substance abuse treatment beds
- The number of days in the quarter that the hospital or health care facility was at capacity and not able to accept referrals for substance abuse treatment
